Job Description

Day Shift 8 hours/day 5 days/week

• Title: Physical Therapist Home Health
• Location: Moraine
• Duration: 13 Weeks
• Shift: Day (5x8 Hours) 8:00 17:00
• Full-time position. M – F, no weekends required, but flexible scheduling allows weekend visits if clinician chooses.
• The weekly productivity expectation is 7 visits/day or 35 visits per week.
• Geographic location of the position covers Dayton, Brookville, Client, West Alexandria
• 1 year PT experience required
• Patient population – adults, geriatrics
• State licensure required
• CPR required
• Home Care Home Base is EMR system used – facility will provide training if candidate does have prior experience with this system
