Physical Therapist – Home Health | Travel Contract

Position Overview

As a Home Health Physical Therapist, you will evaluate and treat patients in their homes, helping restore mobility, improve function, and enhance quality of life. You’ll collaborate with interdisciplinary teams while delivering compassionate, evidence-based care.

Key Responsibilities

Perform comprehensive patient evaluations and assessments

Develop individualized treatment and discharge plans

Deliver therapy interventions using appropriate clinical techniques

Complete accurate and timely clinical documentation

Supervise Physical Therapist Assistants as assigned

Collaborate with physicians, nurses, and interdisciplinary teams

Requirements

Experience

Minimum 1 year of recent Home Health Physical Therapy experience REQUIRED

Must have experience with:

OASIS documentation

Education

Doctor of Physical Therapy (DPT) required for licenses obtained January 2016 or later

Certifications & Licensure

Active California Physical Therapy license

BLS or CPR certification from an approved provider
