Eastside Rehabilitation Hospital seeks a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate and treat patients with diverse conditions, developing individualized rehabilitation plans to restore function, mobility, and independence. Collaborate closely with an interdisciplinary team, educate patients and families, and document progress. Evaluate patients with a variety of medical and post-surgical conditions to assess functional limitations, mobility, strength, balance, and pain.
Develop and implement individualized, evidence-based treatment plans focused on improving mobility, function, and quality of life.
Provide hands-on interventions including therapeutic exercise, gait training, balance training, and manual therapy techniques as appropriate.
Educate patients and their families on diagnosis, treatment goals, safety, and home exercise programs to support continued progress after discharge.
Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other team members to coordinate comprehensive patient care plans.
Regularly reassess patient progress, adjust treatment plans, and document all care in the electronic medical record in a timely and accurate manner.
Participate in interdisciplinary team meetings, case conferences, and discharge planning to support safe transitions of care.
Adhere to all regulatory, ethical, and professional practice standards and maintain compliance with hospital policies and procedures.

- Current Doctor of Physical Therapy (DPT) or professional degree in Physical Therapy from an accredited program
- Active, unrestricted Physical Therapist license in the state of practice
- Basic Life Support (BLS) certification
- Clinical experience in inpatient rehabilitation, acute care, or related setting
- Preferred: experience with neurological, orthopedic, and medically complex rehabilitation populations
- Preferred: specialty certification such as Neurologic Clinical Specialist (NCS), Orthopaedic Clinical Specialist (OCS), or Certified Strength and Conditioning Specialist (CSCS), if applicable
