Physical Therapist Case Manager, Level 2: Home Health

Full Time Physical Therapist Case Manager, Level 2
Anticipated Territory: Windsor, CA Area

Responsibilities

• Perform evaluations in patients’ homes and develop individual care plans.
• Work with Physical Therapy Assistants (PTAs) to follow care plans.
• Help patients improve mobility and regain independence.
• Communicate and coordinate with physicians to ensure quality of care.
• Evaluate and report patients’ progress to medical directors.
• Supervise and mentor 2 PTAs, ensuring they meet performance standards.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License.
• Knowledge of local community resources required.
• Understanding of illness/injury impact on development and behavior, including cultural influences on healthcare.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home‑health care highly preferred.
• Experience with high‑acuity patients a plus.
• Experience working with an interdisciplinary team highly desired.
• Case management fundamentals desired.
• Self‑directed, empathetic, critical thinker with strong interpersonal and leadership skills and organized.

• Wage range: $125,000.00 – $160,000.00 plus $20,000 annual bonus, paid quarterly.
