Physical Therapist Case Manager: Home Health

21st Century Home Health Services (21HHS) is a leading home health agency in the Bay Area, caring for over 4,000 patients across multiple counties.

Base pay range: $120,000.00/yr – $135,000.00/yr. Tiered bonus program can increase compensation by up to $10,000 annually.

Full‑time, on‑site field role visiting patients in their homes.

Key Responsibilities

• Perform home visits to evaluate needs and develop individualized care plans.
• Conduct physical therapy sessions, supervise PTAs, and monitor patient progress.
• Coordinate with physicians, nurses, and allied health staff to ensure quality, continuity of care.
• Document assessments, treatment sessions, and progress reports in the electronic medical record.
• Provide patient and family education on self‑management, home modifications, and safety.
• Assist with administrative tasks and documentation to maintain billing accuracy.

Qualifications

• Current unrestricted California Physical Therapist license.
• BLS/CPR certification.
• Valid California driver’s license.
• Experience in home health and high‑acuity patient care preferred.
• Knowledge of local community resources and state/federal home‑care regulations.
• Strong case‑management fundamentals and interdisciplinary teamwork experience.
• Self‑directed, empathetic, critical thinker with excellent interpersonal and leadership skills.
