Physical Therapist Case Manager: Home Health

Full Time Physical Therapist Case Manager

Location: Solano County – Vacaville*
Field visits are required; remote work is not available unless specifically noted.

In partnership with other clinicians, you will evaluate, treat, and manage patients in their homes to help them live safely and productively.

Responsibilities

• Perform an evaluation in the patient’s home and develop a care plan.
• Work with Physical Therapy Assistants (PTAs) to follow the care plan.
• Help patients with mobility, movement, and regaining independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to the MD.
• Assist with documentation and administrative tasks (concierge service).

Requirements

• Current unrestricted California Physical Therapist License.
• Current BLS/CPR Certification.
• Valid California Driver’s License.
• Knowledge of local community resources.
• Understanding of cultural influences on health care.
• Familiarity with state and federal homecare regulatory guidelines (preferred).
• Experience in home health care (highly preferred).
• Experience with high acuity patients (a plus).
• Experience working with an interdisciplinary team (highly desired).
• Case Management fundamentals (highly desired).
• Self‑directed, empathetic, and critical thinker.
• Outstanding interpersonal, leadership, and organizational skills.
