Physical Therapist Case Manager: Home Health

Full Time Physical Therapist Case Manager

Anticipated Territory: Sacramento County: Elk Grove and Surrounding cities*

Responsibilities

• Perform home evaluations and develop individualized care plans in collaboration with Physical Therapy Assistants.
• Assist patients with mobility, movement and regaining independence.
• Create and update detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patient progress to the supervising physician.
• Assist with documentation and administrative tasks as part of the concierge service.
• Create a flexible work schedule and manage time within targeted zip code territories.
• Maintain autonomy with no clock‑in/out system.
• Operate within the company’s technology stack, including provided devices.
• Receive mileage reimbursement at $0.70 per mile between visits.

Qualifications

• Unrestricted California Physical Therapist license.
• BLS/CPR certification.
• Valid California driver’s license.
• Knowledge of local community resources.
• Understanding of individual development, human behavior and cultural influences on health care.
• Familiarity with state and federal home‑care regulatory guidelines (preferred).
• Experience in home health care (highly preferred).
• Experience with high acuity patients (plus).
• Experience working with interdisciplinary teams (highly desired).
• Case‑management fundamentals (highly desired).
• Self‑directed, empathetic, critical thinker.
• Outstanding interpersonal, leadership and organizational skills.
