Responsibilities may include:Assessing and evaluating therapeutic, rehabilitative, and functional status, including home environmentAssessing muscle strength, mobility, gait, range of motion, and potential for rehabilitationDirecting and implementing physical therapy treatmentsInstructing patients and families/caregivers in the use and care of therapeutic appliancesDetermining priority needs for physical therapyReporting patient reactions to treatment or changes in condition to the physicianInitiating physical therapy programs and instructing other personnel or family/caregivers as neededTraining patients in the use of prosthetic devicesIdentifying patient and caregiver needs for other home health services and making referrals as appropriatePreparing and submitting clinical and progress summaries based on goal attainmentParticipating in patient discharge planningProviding physical therapy consultation to families/caregivers when indicatedProviding in-service education programs for nursing personnel as neededParticipating in peer consultationSupervising physical therapy assistants according to organizational policy and state regulationsInstructing, supervising, and evaluating home health aide care every two weeks when therapy is the only skilled serviceThe above statements are intended to represent the major duties and responsibilities of this job.
