About this Position

Iconic Care is a physician‑owned group of skilled providers (physicians, PAs, and NPs) offering care across ten states with plans to expand nationwide. Our mission is to improve the quality of life and therapeutic outcomes for patients in skilled nursing and long‑term care facilities by collaborating closely with primary care teams and enhancing patients’ participation in physical therapy while reducing reliance on pain medications.

Day‑to‑Day Responsibilities

As a physiatrist, you will round at 1–3 skilled nursing facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who have difficulty participating in therapy due to pain or functional limitations.

• Perform focused assessments and create individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
• Serve as the sole physiatrist on‑site, operating independently while supported by a collaborative clinical team for consultation as needed.
• Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Experience in nursing home or physiatry settings is a plus, but not required.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.
