- On-call required (up to 3 nights per week)
- Weekend coverage: Every third weekend
- Provide inpatient pharmacy services supporting acute care patients across all age groups, including neonates through geriatrics
- Perform clinical dosing and medication management , including anticoagulation, renal dosing, pharmacokinetics, therapeutic drug monitoring, and high-alert medications
- Prepare and dispense IV medications , including IV piggybacks, IV push medications, antibiotics, and chemotherapy agents
- Compound medications using proper sterile technique and safe handling of hazardous drugs and antineoplastic agents
- Support outpatient infusion and chemotherapy preparation as needed
- Collaborate with interdisciplinary teams to ensure safe and effective medication therapy
- Utilize Epic for order verification, documentation, and workflow
- Float within scope of practice to meet facility needs
- Minimum 2 years of inpatient pharmacist experience
- PharmD or Bachelor's Degree in Pharmacy
- Active state pharmacy license
- RPh credential
- Experience with Epic
- Experience preparing IV medications and supporting acute care pharmacy operations
- Ability to take on-call shifts and weekend coverage
- Experience with inpatient pharmacy duties and outpatient infusion preparation
- Chemotherapy/Hazardous Drug Certification
- IV Certification
- Experience with chemotherapy compounding and antineoplastic drug packaging
- Experience in ambulatory or outpatient clinic pharmacy settings
- Pediatric dosing experience
