nPharmacy Medication Reconciliation Technician

nThe Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.
nDescription:
nSupports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers
nInformation obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
nObtains an accurate medication and allergy history by interviewing the patient, family, pharmacy, long term care facility, and/or other providers.
nScreens and reviews patient medications for problems related to duplications, contraindications due to allergies or new conditions, NPO status, and communicates problems to the pharmacist.
nAccurately creates a medication reconciliation list that contains the drug name, dose, strength, frequency, and route for all prescription, over the counter, herbal, and professional samples that the patient is taking, and validates how the patient takes their medications.
nDocuments changes in the historical medication list within the EMR.
nRequirements:
nLess than High School.
n6 months of pharmacy experience required.
nNationally Certified as pharmacy technician.
nRequired Kentucky Board of Pharmacy registration certificate for Pharmacy Technician
