Pharmacy Clinical Specialist - Internal Medicine/ Pulmonary (Rotating)

Coordinates and provides the pharmaceutical care of patients for an assigned clinical unit and participates in patient care rounds with medical staff personnel.

The Cleveland Clinic internal medicine services provide an exciting environment for high quality patient care for a high acuity patient population. Daily responsibilities include decentralized coverage of assigned units and teams, coordination of pharmaceutical care incorporating multidisciplinary involvement, therapeutic drug monitoring, minimization of safety issues, ensuring cost effective pharmacotherapy plans, and medication reconciliation. Collaboration with other healthcare professionals is imperative.

Cleveland Clinic’s inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians, supporting 60 nursing units, 80 operating rooms, 4 PACUs, and 10 cath labs, accounting for over 1,440 inpatient beds.

We are recruiting a highly motivated pharmacist to provide patient care in our internal medicine and subspecialty areas, rotating between 2‑3 teams. Other responsibilities include precepting APPE students and PGY1/PGY2 residents, participation in projects/research work that supports the department’s mission, and representing pharmacy on appropriate committees.

Practice model / patient population

• Direct patient care responsibilities rotating between 2–3 internal medicine services or subspecialties.
• There are 9 internal medicine teaching teams.
• Subspecialties include hepatology, urology, and pulmonology.
• Decentralized service coverage: ~70%; centralized coverage: ~30%.
• Pharmacists work every 8th weekend, rotating between decentralized internal medicine coverage (day) and centralized coverage within the main pharmacy (evening).
• Other scheduling items: 8 weeks of evening shift coverage per year, rotating between the decentralized internal medicine position and centralized coverage in the main pharmacy; assigned one holiday per year; part of the night shift coverage rotation (approximately once every 18–24 months).

A caregiver in this position works days/evenings from 7:00 AM–3:30 PM or 1:00 PM–9:30 PM, with weekend availability.

A caregiver who excels in this role will:

• Coordinate and provide pharmaceutical care for patients.
• Participate in patient care rounds.
• Perform pharmacotherapeutic monitoring and provision of pharmaceutical care.
• Provide general and patient-specific drug information.
• Educate clinical pharmacists, technicians, pharmacy students, post‑graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy.

Minimum qualifications

• PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
• If hired for a specialty area where specialty residency training is available, then specialty residency is required.
• Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire.
• Completion of a post‑graduate year 2 residency (PGY2) or fellowship OR completion of a post‑graduate year 1 residency (PGY‑1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring.
• For individuals required to administer immunizations: current Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire.
• For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or certification obtained within 1 year of hire.

• Board Certification.

Physical Requirements

• Typical physical demands include ability to stand and walk for prolonged times, approximately 75% of shift.
• Manual dexterity sufficient for computer and dispensing functions; ability to lift up to 30 lbs.
• Position often includes reaching, stretching, stooping, climbing steps and sitting.
• Must have normal or corrected vision.
• Must have ability to clearly communicate verbally by phone or in person.

Pay range

Minimum Annual Salary: $101,900.00

Maximum Annual Salary: $155,390.00
