Pharmacist – Remote Medication Therapy Management (MTM) Pilot Program (California Residents Only)

This remote, part-time opportunity is designed for California-licensed Pharmacists interested in supporting a Medication Therapy Management pilot program aimed at enhancing clinical pharmacy services. Working 10-20 hours per week during day shifts, you will provide patient counseling, conduct MTM consultations, and help optimize medication regimens—all from your home within California. Candidates must reside in California and be currently enrolled in Medicaid as an Ordering, Referring, and Prescribing (ORP) Provider.

Location:

• Remote work within California (CA residency required)

Schedule:

• Flexible 10-20 hours per week
• Day shift hours required for patient outreach and counseling
• Guaranteed minimum of 8 paid hours weekly regardless of workload

Responsibilities:

• Conduct comprehensive Medication Therapy Management consultations remotely
• Provide patient counseling and medication education to support adherence and health outcomes
• Review medication profiles to identify opportunities for optimization and minimize adverse effects
• Accurately document clinical interventions and patient encounters
• Collaborate closely with program stakeholders to support clinical goals and improve patient outcomes
• Maintain strict compliance with all applicable state and federal pharmacy regulations

Qualifications:

• Active, unrestricted California Pharmacist license
• Current California resident
• Previous experience providing MTM services
• Enrollment in California Medicaid (Medi-Cal) as an ORP Provider REQUIRED
• Excellent communication and patient counseling skills
• Self-motivated with the ability to work independently in a fully remote setting
