About the Position
The Pharmacist is responsible for the safe and effective use of medication throughout the health system.
Location: 7300 Pharmacy
Schedule: PRN as needed
Overview of Responsibilities
- Reviews and interprets provider orders and performs clinical checking.
- Communicates clinical concerns such as allergies, drug interactions, side effects, and min/max warnings to providers.
- Monitors patient medication therapies and provides interventions.
- Approves all medications and IV’s before they are dispensed.
- Counsels patients and caregivers on medication use.
- Works proficiently in all pharmacy information systems.
- Prepares medications during Code Blue situations.
- Participates in adverse drug reaction review and reporting.
- Documents all clinical activities.
- Performs thorough technician evaluations.
- Participates in QA&I program or other related projects.
- Participates in antimicrobial stewardship activities (if applicable).
- Provides thorough shift-to-shift communication.
- Reports medication errors – prevented and actual.

Education & Experience
- Minimum education required: Bachelor of Pharmacy degree.
- Preferred education: Pharm D degree from an ACPE accredited college of pharmacy.
Licensure & Certification
- Active pharmacist license in the state of Indiana.
