In this on-site role, you will provide patient-centered medication therapy management, verify and dispense prescriptions, and counsel underserved patients on safe, effective, and affordable medication use. Working closely with primary care, dental, behavioral health, and wellness providers, you will optimize chronic disease management, support 340B and access-to-medication programs, and advance whole-person, faith-informed care.
