A California-licensed pharmacist residing in California is sought to provide remote Medication Therapy Management (MTM) services as part of an innovative clinical pilot program. This part-time contract role offers flexible scheduling with approximately 10-20 hours per week, primarily during day shifts, supporting patient outreach and counseling efforts from the comfort of your home.
Key Responsibilities:

• Conduct MTM consultations to improve patient medication outcomes
• Provide clear patient counseling and medication education
• Review medication profiles to identify and recommend optimization opportunities
• Accurately document clinical interventions and patient interactions
• Collaborate with stakeholders to enhance program success and patient care
• Ensure compliance with all relevant state and federal pharmacy regulations

Qualifications and Experience:

• Active, unrestricted California Pharmacist license
• Current residency within California
• Previous MTM experience is essential
• Enrollment as an Ordering, Referring, and Prescribing (ORP) provider with California Medicaid (Medi-Cal) is mandatory
• Excellent communication and patient counseling abilities
• Comfortable working independently and managing flexible remote hours

Schedule:

• Approximately 10-20 hours weekly, flexible day shift hours to align with patient needs
• Guaranteed minimum of 8 paid hours per week regardless of workload
