Responsibilities

• Provide consultation for other health care providers.
• Educate patients and other health care providers.
• Review and process medication orders, and prepare and dispense medication.
• Participate as an active member of interdisciplinary care teams.
• Advise on the most efficacious treatments and participate in pharmacotherapeutic decision making.
• Participate in collaborative practice protocols.
• Provide virtual pharmacy consults to Carilion Clinic Ambulatory Offices.
• Assist with drug information and evidence‑based medicine.
• Consult and provide healthcare providers advice on medications.
• Respond to patient emergencies when required.
• Review and verify medication orders for accuracy.
• Ensure preparation of sterile/nonsterile compounding per Standard Operating Procedures.
• Ensure timely delivery of medications.
• Provide education and training for healthcare providers, students, residents, patients, and families.
• Provide feedback to pharmacy students/residents.
• Maintain competency in all technologies used by the Department of Pharmacy.
• Support clinical pharmacy initiatives of the institution.
• Maintain compliance with statutes, laws, regulations, standards and policies governing pharmacy practice.
• Facilitate age‑specific pharmaceutical care to patients.

Requirements

• Graduate of a College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE).
• Pharmacy Practice Residency or equivalent hospital experience is preferred.
• License to practice pharmacy in the Commonwealth of Virginia required within 90 days of hire.
• Good leadership, communication, interpersonal and teamwork skills.
• Ability to budget time, work efficiently and accurately, following procedures with minimum supervision.
• Excellent critical thinking and analytical skills.
• Proficient in general office equipment and procedures, including computer and keyboarding skills.
