Summary

Utilizing the nursing process, the RN provides acute and long‑term nursing care to patients, families and the therapeutic milieu within the Program, Department and Hospital standards. Responsibilities

• Provide direct nursing and patient care services by assessing patients’ immediate and long‑term needs.
• Initiate a biopsychosocial assessment of patients’ acute nursing needs guided by the admission procedure; document in required patient medical records format.
• Ensure completion of the nursing assessment form within 8 hours of admission for each assigned patient.
• Communicate significant changes in assessment findings to the nurse in charge and other health‑care providers, and summarize patient status in team meetings and reports.
• Plan treatment interventions with patient, family and health‑care team based on the nursing process.
• Initiate and complete a patient care plan within 8 hours of admission in accordance with the admission procedure.
• Plan for patient/staff safety according to established hospital procedures.
• Develop and update the patient care plan each shift to reflect patient status.
• Formulate patient and family treatment goals; identify nursing interventions based on those goals.
• Identify patient/family strengths and inpatient and after‑care needs.
• Formulate measurable short‑ and long‑term goals with timelines.
• Maintain therapeutic alliance with patients and families, reflected in documentation and supervision.
• Advocate for patients by supporting the Bill of Rights and child mental‑health law.
• Acquire and maintain technical and psychiatric specialty skills appropriate to the clinical program.
• Implement interventions as specified in the Nursing Care Plan within established time frames.
• Follow policies, procedures, program protocols and The Joint Commission standards to ensure safe practice.
• Provide patient education regarding procedures before implementation.
• Administer and document prescribed medications (scheduled/PRN) according to protocol.
• Assess and document medication effectiveness and side‑effects.
• Transcribe physician orders accurately and implement them as prescribed.
• Evaluate the effectiveness of care provided and modify interventions as needed.
• Seek consultation and guidance through established supervision lines to resolve issues affecting patient outcomes and relationships.
• Participate in quality‑assurance data collection and contribute to the Program‑based quality‑assurance program.
• Communicate patient status through behaviorally oriented procedures.
• Question medical orders that conflict with ethical or patient‑care standards.
• Participate in ongoing development and implementation of the therapeutic milieu.
• Provide milieu management each shift by assessing and responding to acute patient‑group needs, including safety, group interactions and staff interactions.
• Respond promptly to assistance requests beyond the unit level as directed by Program protocol.
• Provide leadership during acute or emergency situations; serve as a member of the Emergency Response Team.
• Lead or co‑lead therapeutic milieu groups, supporting individual patient participation.
• Organize and participate in informal patient groups within the program‑based milieu assessment.
• Communicate changes in individual patient/milieu status to the nurse in charge.
• Participate directly in the milieu program as a direct‑care provider.
• Demonstrate leadership in organizing the delivery of patient care.
• Supervise Behavioral Health Specialists and support staff within their scope of practice.
• Prioritize interventions in rapidly changing patient situations, demonstrating organized response to optimize patient welfare.
• Provide consistent leadership on the unit when assigned as nurse in charge.
• Identify and address teaching needs of patients, families and other health‑care providers; document teaching in the care plan.
• Provide basic and complex teaching to patients, documenting responses.
• Assist in orientation of new employees and students according to guidelines.
• Give impromptu instruction and demonstrate therapeutic techniques and nursing procedures.
• Maintain professional growth through ongoing activities promoting skill development.
• Manage time effectively, prioritizing urgent issues while ensuring completion of assigned work.
• Document required materials consistently, keep records, and disseminate to appropriate parties.
• Follow established job schedule and coverage procedures.
• Keep current knowledge of the field; share knowledge during team meetings and informal sessions.
• Participate in performance‑appraisal process, setting specific short‑ and long‑term professional goals with supervisor.
• Stay updated on hospital and program changes through meetings, minutes, and new policies.
• Adhere to hospital, department, and program policies and procedures.
• Attend six program meetings per year as assigned.
• Maintain certification in crisis management and Basic Life Support.
• Attend program staff‑development sessions and monthly development day presentations as assigned.

Minimum Qualifications

• Licensure as a Registered Nurse in the State of Rhode Island (or licensure under the Nurse Licensure Compact).
• Graduate of a school of nursing with current license to practice as a RN in Rhode Island.
• Demonstrated knowledge and skills to provide care to patients considering aging, human development and cultural patterns.
• Preferred: Two years of psychiatric or pediatric nursing experience (manager will review eligibility).

Work Environment and Physical Requirements

Work performed in a psychology‑based clinical setting requiring extensive standing, walking, and occasional lifting of patients. Strong background and experience working with students who have various emotional and behavioral disorders and developmental disabilities, including intellectual disability and autism spectrum disorders.

$35.00-$70.00
