Per Diem / Part-Time RN for the Assisted Living Waiver (ALW) Program | Sacramento County

Are you an RN looking for a flexible, part-time opportunity where you can set your own schedule, work independently, and build one-on-one relationships with clients outside of a traditional hospital setting? Elder Options, Inc. is looking for a Per Diem / Part-Time Registered Nurse to join our Assisted Living Waiver (ALW) team. This is a unique nursing position designed for an RN who wants to use their clinical experience in a more flexible, community-based role. You\'ll work directly with clients living in assisted living facilities, skilled nursing facilities, and home settings throughout Sacramento County while collaborating with our team of nurses, social workers, care managers, and other professionals.

This is a part-time position with a caseload that varies based on program needs (approximately 30-40 clients per month). One of the biggest benefits? You manage your own schedule. There are no traditional nursing shifts. You\'ll receive your assigned caseload each month and independently coordinate your in-person visits throughout the month, allowing you to fit the position around your existing schedule while developing ongoing relationships with the clients you serve.

What You'll Do

Our ALW RNs use their nursing expertise to assess, monitor, and support clients participating in the Assisted Living Waiver Program. Responsibilities include:

  • Conducting in-person monthly monitoring visits with assigned clients
  • Completing initial nursing assessments (as assigned) and semi-annual reassessments (scheduled)
  • Assessing and evaluating client needs, services, changes in condition, and overall wellbeing
  • Updating assessments, Individualized Service Plans (ISPs), narratives, and other program documentation accurately
  • Entering timely case notes and additional documentation into company-specific software
  • Maintaining an accurate weekly schedule of completed client visits
  • Developing supportive relationships with clients, families, and conservators and communicating relevant updates and/or concerns following visits
  • Developing positive professional relationships with facility administrators and staff
  • Promptly reporting client absences, incidents, changes in condition, deaths, or other concerns to the Elder Options ALW team
  • Collaborating with Elder Options office staff to address client, family, and facility needs
  • Completing assigned visits and documentation within established monthly deadlines (by the 23rd of each month)
  • Participating in mandatory in-person bi-monthly staff meetings
  • Completing 10 hours of required training annually

What We're Looking For

We\'re looking for an RN who is comfortable working independently and values the opportunity to get to know clients beyond a single visit or hospital stay. Requirements include:

  • Current California Registered Nurse (RN) license in good standing
  • Minimum of 3 years of nursing experience
  • Strong assessment and evaluation skills
  • Strong organization and time-management skills
  • Ability to independently manage a monthly caseload and meet required deadlines
  • Comfortable completing electronic documentation and navigating various software systems
  • Ability to maintain client confidentiality and comply with HIPAA and all applicable program requirements
  • Reliable transportation and ability to travel throughout Sacramento County and attend bi-monthly Staff Meetings in Placerville
  • Ability to work collaboratively with clients, families, facility staff, and the Elder Options team to provide each client with ongoing, individualized, person-centered care

Experience with geriatrics, assisted living, skilled nursing, community-based care, Care Management, or individuals with disabilities is not required, but a huge plus!

Training Provided

Don\'t have ALW experience? That\'s okay! Training for this specialized program and our documentation processes is provided. We\'re looking for an experienced RN with strong clinical judgment, reliability, and a willingness to learn the program.
