Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in members' homes, bridging gaps in care across Fort Collins and surrounding areas.
You’ll conduct 45‑minute visits, review medications/history, complete a physical, and perform optional diagnostic tests while using an iPad workflow. Travel within licensed states is required.
