Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in members’ homes. You’ll conduct health assessments, history reviews and may perform additional tests as ordered by the plan.
The role offers a part-time schedule (about 12 hours/week) with travel within licensed states and occasional virtual visits. Base pay is hourly, with benefits for eligible part-time employees.
