Responsibilities:
- Provide comprehensive end-of-life nursing care in a home health setting.
- Assess and manage patient symptoms, including pain and distress.
- Develop and implement individualized care plans.
- Document care activities accurately within electronic medical records.
- Collaborate with interdisciplinary teams to ensure quality patient care.
- Educate patients and families on hospice services and end-of-life procedures.
