Lamoille Health Partners , a Federally Qualified Health Center (FQHC) committed to providing comprehensive, patient-centered care to underserved populations, is seeking a dedicated and compassionate Family Medicine Provider to join our team.

Responsibilities
- Cover for providers who are on leave and assume responsibility for panel management and patient visits
- Provide comprehensive primary care services to patients of all ages, including preventive care, acute care, and chronic disease management.
- Conduct thorough patient assessments, develop individualized treatment plans, and provide patient education.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures.
- Collaborate effectively with other members of the healthcare team, including physicians, nurses, medical assistants, and behavioral health providers.
- Maintain accurate and timely patient records in accordance with HIPAA and other regulatory requirements.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning.
Qualifications
Required
- MD, DO, NP or PA degree from an accredited program
- Current and unrestricted license to practice in Vermont.
- 5 years or more of experience working in a health center or hospital setting preferred.
- Current BLS certification.
- Excellent communication and interpersonal skills.
- Strong clinical and problem-solving skills.
- Commitment to serving underserved populations.
- Ability to work effectively in a team-based environment.
Preferred
- Experience working in an FQHC or community health center setting.
- Experience with electronic health records (EHR) systems, specially eClinicalWorks.
