Per Diem Case Manager - Inpatient Post-ICU Care

Select Specialty Hospital in Wyandotte, MI, is seeking a PRN Case Manager to support post-ICU recovery for chronically critically ill patients. Requires current licensure (RN/LPN/LVNs or RT) or Social Work SW/MSW, and prior discharge planning experience.

You will handle utilization reviews, discharge planning, and care coordination, reporting to the Director of Case Management, with weekday day shifts available on an as-needed basis.
