Pennsylvania Licensed Behavioral Health Case Manager

To support members facing behavioral health challenges, the full-time Pennsylvania Licensed Behavioral Health Case Manager will engage clients remotely to develop personalized care plans, provide advocacy, and facilitate access to resources while collaborating with healthcare teams. Key Responsibilities: Telephonically engage members to identify barriers to care related to Social Determinants of Health (SDOH) Develop care plans by assessing members' strengths, needs, and goals, providing follow-up as clinically appropriate Coordinate with healthcare teams to address complex behavioral health situations and ensure effective service delivery Required Qualifications: Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required Minimum of 3-5 years post-licensure experience in an integrated care or healthcare setting Strong understanding of Social Determinants of Health and Motivational Interviewing techniques Ability to work independently and resolve complex behavioral, medical, and psychosocial issues Exceptional assessment, engagement, and organizational skills with proficiency in Microsoft Office Suite
