Providing outreach to patients with complex medical and social needs, the full-time Pennsylvania Certified Community Health Worker will assist patients in navigating the healthcare system while working remotely. Key responsibilities Orients and assesses new Community Health Workers and evaluates their proficiency during the training period Engages with patients through various means, including phone calls, home visits, and accompanying them to provider appointments Identifies gaps in care, connects members with resources, and facilitates adherence to treatment plans Required qualifications High School Diploma or Equivalent (GED) - Required Minimum of 1 year of related work experience - Required Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board - Required
