Peer Recovery Navigator
Under the direction and supervision of a Qualified Health Professional, the Peer Recovery Navigator, a CARC or CPRA will provide concrete services geared toward assisting patients with behavioral health needs specific to the need for induction of Buprenorphine (Suboxone). The Peer Recovery Navigator will provide peer support to engage the individual on a continuum of care to help achieve and maintain motivation for continuing their care while in recovery from substance use and mental illness.
Conduct in person outreach to patients having difficulty engaging in behavioral health treatments
Outreach phone calls and letters
Accompany patients to treatment related appointments
Make home or agency related visits
Promote pro-social behaviors
Document and bill all interaction recorded in the electronic health records (EHR)
Adhere to confidential policy and HIPPAA protocol as related to patient's information/interactions
Attend all agency staff meetings and annual in-service as required or mandated
Maintain good working relationships with all agency staff, patients and visitors
High School Diploma or GED
CARC or CRPA
Prior experience working with the target population and lived experience with Buprenorphine/Suboxone
Experience and knowledge of Microsoft systems and programs
Excellent written and oral communication
Ability to speak, read and write the English language
Bilingual English/Spanish a plus
Must possess sight, hearing and digital senses or use prosthesis that will enable the essential functions of the job to be completed.
