VNA Health Care is seeking a compassionate and dedicated Pediatric Nurse Practitioner (PNP) to provide comprehensive, patient-centered primary care to infants, children, and adolescents. As an integral member of our multidisciplinary care team, you will promote healthy growth and development while delivering high-quality preventive, acute, and chronic pediatric care in a community health setting.

Responsibilities

  • Provide comprehensive health assessments, well-child examinations, and developmental screenings for pediatric patients from birth through adolescence.
  • Diagnose and manage acute and chronic pediatric illnesses and injuries.
  • Order, interpret, and communicate laboratory and diagnostic test results.
  • Prescribe medications and develop individualized treatment plans in accordance with Illinois APRN regulations.
  • Administer and promote recommended childhood immunizations.
  • Educate patients and families on growth and development, nutrition, safety, preventive care, and disease management.
  • Collaborate with physicians, behavioral health providers, nurses, care coordinators, and other members of the healthcare team to ensure coordinated, high-quality care.
  • Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
  • Participate in quality improvement initiatives and support VNA Health Care's mission of providing accessible, equitable healthcare to the community.

Qualifications

  • Master's or Doctor of Nursing Practice (DNP) degree from an accredited Pediatric Nurse Practitioner program.
  • Current Illinois Advanced Practice Registered Nurse (APRN) license.
  • National certification as a Pediatric Nurse Practitioner (PNP-PC or PNP-AC).
  • Current BLS certification.
  • Minimum of one year of pediatric clinical experience preferred.
  • Excellent communication, clinical decision-making, and interpersonal skills.
  • Bilingual (English/Spanish) preferred but not required.
