Requirements:

• Active New Mexico Medical License (required at the time of submission)
  
• Board Certified or Board Eligible (within 3 5 years of residency)
  
• Active DEA
  
• Current BLS, PALS, ACLS, and ATLS certifications
  
• Current CV required for credentialing
  

Credentialing:

• Estimated credentialing timeframe: days
  
• Providers must be committed to completing the privileging application within one week of receipt to avoid delays.
