The Staff Pad is proud to partner with a respected hospice provider in the DFW Metroplex, seeking acompassionate Pediatric RN Case Manager to provide exceptional hospice care tochildren and their families in the comfort of their homes.

Reporting to the Managing Director - Hospice, the Pediatric RN Case Manager, will provideskilled nursing care to pediatric hospice patients while coordinating allaspects of their care. Working alongside an interdisciplinary team, physicians,and community partners, you'll help ensure every child receives compassionate,dignified, and personalized care.

What You’ll Do

Key Responsibilities

  • Conductcomprehensive physical, psychosocial, and environmental patientassessments
  • Develop,implement, and document individualized plans of care
  • Communicateeffectively with patients, families, physicians, and the care team
  • Provideclinical guidance to Hospice Aides and LVNs and assist with training asneeded
  • Completepediatric admissions and prioritize patient scheduling
  • Participatein assigned on-call rotations for emergency patient support
  • Maintainaccurate documentation and ensure compliance with infection control andagency policies
  • Supportongoing quality improvement and professional development
  • Performother duties as assigned

What We Are Looking For

Qualifications

  • Graduateof an accredited Diploma, ADN, or BSN Nursing program
  • CurrentTexas Registered Nurse (RN) license
  • Minimumone (1) year of pediatric RN experience in a clinical setting required
  • Hospice,Critical Care, or Emergency Room experience preferred
  • CurrentCPR certification
  • Validdriver's license, reliable transportation, and current auto insurance
  • CurrentTB testing and Hepatitis consent/declination documentation
  • Abilityto pass a criminal background check
