Pediatric Care Transitions Nurse Navigator

CHOC is seeking a Discharge Nurse Navigator to support safe transitions from hospital to home. The DNN conducts daily rounds, identifies discharge barriers, and collaborates with the care team to optimize planning and patient education.

Responsibilities include coordinating education on diagnosis, plan of care, medications, and home care needs, using teach-back techniques with families, and participating in quality improvement activities. CA RN required with pediatric experience.
