Pediatric ARNP — Primary Care in Value-Based Model

Nuvia Medical in Coconut Creek, FL is seeking an Advanced Registered Nurse Practitioner to provide outpatient primary care under a value-based model. The ARNP works autonomously and with physicians to manage acute and chronic conditions, coordinate preventive services, and promote health outcomes for a defined patient panel.

You will collaborate with care coordination staff and QA teams to close gaps in care, meet HEDIS and Continuity-of-Care (COC) measures, and reduce unnecessary ER visits.
