Providing essential support to improve patient engagement and care navigation, the full-time Patient Navigator will assist patients with appointment scheduling, transportation coordination, and access to community resources in a remote work environment. Key responsibilities Connect with patients via phone, SMS, email, or IVR to encourage engagement in care and support navigation through healthcare pathways Provide live member support, including appointment scheduling and coordination of services, while ensuring compliance with HIPAA regulations Track patient needs to resolution, documenting interactions and monitoring issue trends for continuous service improvement Required qualifications High School Diploma or G.E.D required; an Associate's degree in healthcare or a related field preferred Minimum of two years' experience in customer service, healthcare navigation, or patient support required Preferred training or certification as a Medical Assistant, Social Worker, Community Health Worker, or Patient Navigator Experience in care coordination or accessing community resources preferred
