Patient Navigator - New York Licensed

To support the colorectal cancer screening program, the full-time Patient Navigator - New York Licensed will oversee a caseload of patients, acting as the primary liaison to facilitate colonoscopy scheduling and completion while working in a flexible hybrid schedule. Key responsibilities Schedule and navigate direct access colonoscopies for patients referred to The CHOICE Program Establish relationships with patients to ensure commitment to completing their colonoscopy and address any questions or concerns Train and mentor less experienced Patient Navigators while tracking patient encounters in the CHOICE tracking database Required qualifications Bachelor's degree preferred or High School Degree with relevant work experience Minimum of 2 years experience, with 3-5 years highly preferred Experience as a Patient Navigator or in a similar position Bilingual in Spanish preferred Experience with EMR systems, preferably EPIC
