Ascension is seeking a Registered Nurse Patient Navigator to guide patients and families through complex care journeys. In this role, you will coordinate appointments, facilitate referrals, provide education on diagnoses and treatment plans, and advocate for patient needs across multidisciplinary teams. You will assess barriers to care, support transitions between inpatient and outpatient settings, and ensure continuity of care.
Working in a compassionate, patient-focused environment, you’ll leverage evidence-based nursing practice, electronic health records, and strong communication skills while contributing to quality improvement and education initiatives.
