Patient Care Navigator I

The Patient Services Coordinator at Mount Sinai Health System supports seamless patient care delivery in the New York area. You will liaise with Primary Care Providers and outpatient services, educating patients on care plans and coordinating services to ensure smooth visits.

You will work with clinical staff to move care plans forward, arrange appointments, monitor visit statuses, handle pre-certifications, and route telephone encounters to the appropriate Providers.
