Patient Care Coordinator - Florida

To support a growing mobile dermatology practice, the full-time Patient Care Coordinator - Florida will manage practitioner schedules, maintain relationships with senior communities, and ensure accurate patient information documentation while working onsite. Key responsibilities Organize and maintain monthly provider calendars and communicate visit schedules to communities and providers Develop strong relationships with key decision makers in facilities to facilitate service growth and address concerns Coordinate patient appointments, follow-ups, and referrals, ensuring seamless collaboration with practitioners and internal teams Required qualifications High School Diploma (or equivalent) Prior healthcare experience in patient care coordination or scheduling Strong understanding of insurance eligibility using Availity and Payor specific platforms Proficiency in navigating medical software and EHR systems Compassionate mindset with a genuine desire to prioritize patient care
