Position Title : Patient Care Advocate
Work Location : Remote -Nationally sourced but must work PST hours
Assignment Duration: 2 months (possibility of extension or conversion)
Work Schedule : 8 hours per day M-F 8-5 PST No overtime

Job Description:
The team's purpose is to make outreach calls to members, educate them on the importance of preventative care and screenings. Address all member needs and create positive experiences.
Complete member outreach calls to close care gaps for Medicaid.
Educate members on the importance of preventative care. Focus on improved member experience. Work with other departments to resolve member concerns. Outreach call goals - above 95 call per day. 14-16 appointments made/confirmed per day. Candidate Requirements Education/Certification Required: Bachelors Degree- see below Preferred: Licensure Required:N/A Preferred: Years of experience required: Required: a bachelor's degree in healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field or equivalent work experience required (a total of 4 years of experience required for the position); work experience should be in direct patient care, social work, quality improvement or health coaching preferably in a managed care environment. 2+ years of experience work experience should be in direct patient care, social work, quality improvement or health coaching, preferably in a managed care environment.
Disqualifiers: no remote experience
Additional qualities to look for: Member outreach experience. HEDIS knowledge, high volume outbound call
