Part Time/Weekend Physical Therapist Case Manager

Coverage Area: Elk Grove

In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Your goal as a Physical Therapist is to restore function, prevent or minimize disability following disease or injury, and help the client reach their maximum performance within the limits of their capabilities.

Responsibilities

• Perform a home evaluation and develop a care plan, working with PTAs to follow the plan.
• Help patients with mobility, movement, and regaining independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to MD.
• Assist with documentation and administrative tasks (concierge service).
• Develop and manage your own schedule and hours.
• Accept zip code–based assignments to reduce travel time.
• Work autonomously without clocking in/out.

Qualifications

• Current unrestricted California Physical Therapist License.
• Current BLS/CPR Certification.
• Valid California Driver’s License.
• Knowledge of local community resources.
• Understanding of individual development and human behavior related to health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high‑acuity patients a plus.
• Experience working with an interdisciplinary team highly desired.
• Case‑management fundamentals highly desired.
• Self‑directed, empathetic, critical thinker, outstanding interpersonal and leadership skills, organized.
