Job Title: Part Time Physician Assistant - High Risk Patients

The Role: Ideal candidates are collaborative, compassionate, and thrive in a supportive care team. The Physician Assistant will manage patients in an outpatient clinic, provide in‑home clinical support, prevent unnecessary acute care admissions, and optimize medical and psychosocial care.

What will you do as Physician Assistant at GGH?

  • Assess patients' clinical problems and healthcare needs and provide clinical care and consultation.
  • Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence; obtain and record medical history, symptoms, and medication reconciliation.
  • HCC coding, verification, and risk adjustment.
  • Coordinate continuity of care, prevention and avoidance of complications, and close clinical treatment under the direction of the patient’s primary healthcare provider(s) and with other referral sources.
  • Perform various tests and screenings.
  • Provide counseling around discharge plans.
  • Execute discharge plans and use critical thinking to build effective care plans during transitions after hospital discharge.
  • Support patients and families with compassionate care.
  • Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings with social workers, case managers, and other clinicians.
  • Collaborate with the care team to ensure completion of holistic patient care.
  • Travel within the Jackson County, OR area 5 days a week to provide home‑based care.
  • Complete charts within 72 hours of visits; use EMR and/or portals for chart review and documentation.
  • Cover for other APPs and Care Team Members when necessary.
  • Maintain patient confidentiality and comply with all HIPAA rules and regulations.

  • Be comfortable with technology systems and commit to timely EMR documentation with deep understanding of Medicare HCC coding and managed care.
  • Flexibly adapt to work schedule and setting as needed.
  • Travel within a 35 mile radius of Jackson County, OR 5 days a week to provide care as business needs dictate.

Experience and Qualifications:

  • Current board certified PA.
  • 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care.
  • Acute care experience with high‑risk patients (a plus).
  • Home health experience (PA or RN) (a plus).
  • Working knowledge of chronic care management and medication options.
  • Current DEA licensure / Prescriptive Authority post‑hire.
  • Active CPR/BLS Certification.
