Key Duties
Complete home‑health nursing visits ($30 per visit)
Focus on hypertension and diabetic patient care
Perform assessments, vitals, medication teaching
Provide disease‑management education
Document visits accurately
Communicate patient changes to RN Case Manager
Requirements
Active LPN license in Massachusetts
Home‑health experience preferred
Strong patient‑education and communication skills
Reliable, professional, and able to work independently
