JOB SUMMARY: Provide direct support to adults in community-based (group home) transitional housing, helping them build skills, maintain stability, and work toward personal goals.
- Provide on-site supervision, counseling, advocacy, and goal-focused support to help consumers develop independent living skills, including personal care, household management, budgeting, community involvement, social skills, accessing benefits, and medical coordination.
- Providing safe transportation in an agency vehicle
Respond to crises and contact appropriate staff or authorities when necessary.
- Communicate important updates to the Supervisor and ensure smooth handoff to the next shift.
-Supervise and track medications.
-Assist with collecting service fees.
-Maintain required safety standards, including fire drills, inspections, and reporting facility issues.
-Help keep the environment clean, safe, and secure at all times.
Qualifications:
-Bachelor’s degree in mental health, social work, psychiatric rehabilitation, nursing, or a related discipline; OR a combination of one or more years of college plus work or life experience together equaling four years; OR a High School diploma and four years of related mental health experience.
-Must possess a valid driver’s license, with a good driving record.
