We are searching for an assessment nurse. This is a very part-time yet very important position. The assessment nurse will go to new client's homes, gather and document client's living conditions, health, and personal needs.
The nurse will make a care plan for the Aide to follow. Every client will be reassessed every 60 days, having 2 face to face visits and 4 phone visits.
