Palliative Care Specialist

To support a dynamic healthcare team, the full-time Palliative Care Specialist will manage patient referrals, serve as a primary contact for patients and families, and ensure compliance with healthcare regulations while working remotely. Key responsibilities: Develop and nurture patient referrals while maintaining accurate documentation Serve as a liaison between patients, caregivers, and healthcare teams to facilitate access to care Perform screenings and educational sessions to assess patient eligibility for various levels of care Required qualifications: High School diploma or GED 2 years of experience in Hospice and/or Home Health preferred Knowledge of medical terminology preferred Proficiency in Microsoft Office suite, including Teams Strong customer service skills and ability to manage concurrent demands
