Palliative Care Rn

Job Summary:
- Reviews referrals
- Provides expert, individualized coordination of care for patients and families.
- Completes a comprehensive physical and psychosocial assessment of the patient and family to identify needs and areas for intervention.
- Ensures appropriate referrals to specialists when indicated and supports adherence to a treatment plan, including physician visits for evaluation and ongoing management.
- Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being.
- Assist Nurse Practitioners with day-to-day program tasks and tracking palliative referrals.
- Alleviate pain, fatigue, nausea, and other distressing symptoms.
- Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR.
Job Requirements:
- Requires current RN licensure within state of branch location and three years of clinical nursing experience, preferably in a Home Care, Hospice or Skilled Nursing facility setting. Successfully completed nursing education from an approved school of nursing.
- Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).
- Must accept travel and working conditions as assigned.
- Must possess effective communication skills (oral and written), as well as, CPR certification.
- Minimum of twelve hours of continuing education in a twelve-month period. Must have a valid driver's license.
