Palliative Care Physician Assistant

Job Overview

We are seeking a full‑time (40 hours per week) Palliative Care Advanced Practice Provider for a float position between our centers located in Albuquerque, NM. This position can be filled by a Nurse Practitioner or Physician Assistant. The base salary range for this position is $120,000 – $130,000 per year, in addition to our no‑cap incentive compensation bonus program.

Responsibilities

• Provide primary and urgent care services to nursing center residents in collaboration with other medical providers and Center Leadership.
• Offer palliative care services to residents with serious illness, focusing on pain and symptom management, psychosocial support, and advance care planning.
• Serve as a specialized consultant within the interprofessional team, providing recommendations for care plans that incorporate patient goals and preferences.
• Provide guidance and education to patients, families, and care staff regarding the risks and benefits of interventions to facilitate shared decision‑making.
• Utilize evidence‑based practice and standards of care, including assessment of health status, diagnosis, interpretation of labs and diagnostics, development and implementation of treatment plans, ongoing follow‑up and evaluation, and completion of documentation in accordance with relevant policies and regulations.
• Provide health education to patients and families.
• Demonstrate continued professional competence by maintaining certifications and current knowledge.
• Support the development of nursing staff, collaborate with Center Nursing Executive programs, and deliver targeted clinical education when needed.
• Participate in center‑based quality initiatives, including avoiding hospital transfers, ACO initiatives, and other quality improvement projects.
• Ensure compliance with coding and billing, AlignMed Partners’ Time Off and On‑Call policies, and other assigned duties.
• Implement and support AlignMed Partners’ clinical initiatives.

Qualifications

• Graduate of an accredited School of Nursing, licensed as a registered nurse.
• Certified Nurse Practitioner (or Physician Assistant) with a Master’s Degree in Nursing (or PA program graduate) and concentration/specialty track of Older Adult, Family NP, or an approved track; alternatively, PA with ARC‑PA accreditation, successful PANCE, state licensure, and NCCPA certification.
• Minimum of two years of experience working with adult or geriatric populations (preferred).
• For APRNs, an unrestricted professional license and DEA in each state of practice.

Posted Salary Range

USD $120,000.00 – $130,000.00 per year.
