Palliative Care Nurse, RN

To support a growing virtual palliative care program, the full-time Palliative Care Nurse, RN will engage with patients through telephonic and video consultations, provide compassionate care coordination, and collaborate with interdisciplinary teams to enhance patient outcomes. Key responsibilities Engage with patients to deliver home-based palliative care and conduct routine check-ins based on health status Perform care coordination and case management tasks, including collaborating with home health workers and communicating with primary care providers Participate in team huddles and palliative rounds, while supporting education initiatives for staff Required qualifications Unrestricted RN licensure, with compact licensure preferred A minimum of 5 years of RN experience, including at least 2 years in hospice or palliative care CHPN credential or willingness to obtain within the first six months of employment Ability to work flexibly across time zones as needed Active Basic Life Support (BLS) certification
