To support a growing virtual Palliative Care program, the full-time Palliative Care Nurse Practitioner will manage a panel of high-risk patients, perform assessments, and develop care plans while working remotely across multiple states. Key responsibilities Conduct initial assessments and create Palliative Plans of Care for new patients enrolling in the program Provide ongoing telephonic and video consultations for a panel of approximately 40-50 patients, focusing on advanced symptom management and goals of care Collaborate with interprofessional teams and participate in regular meetings to ensure seamless care coordination and support during periods of growth Required qualifications Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state Minimum of 5 years in advanced practice, with at least 3 years in Palliative Care ACHPN credential or willingness to obtain within the first six months of employment Ability to work across multiple time zones and adapt to a fast-paced, technology-driven environment Experience in interprofessional teamwork and patient-centered care approaches
