Position Summary
As a Palliative Care Nurse Practitioner (NP) with Impact Health PC in Cleveland, OH , you will provide compassionate, patient-centered care to individuals with serious and complex illnesses within skilled nursing facilities. - Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.
- Support goals-of-care discussions with patients and families.
- Collaborate with physicians, facility staff, and interdisciplinary care teams.
- Educate patients, families, and facility staff regarding palliative care and treatment plans.
- Complete timely and accurate clinical documentation.
- Maintain compliance with facility, state, and federal regulations.

- Active Nurse Practitioner (NP) license in MA.
- Minimum 2 years of post-certification experience preferred.
- Palliative care, SNF, hospitalist, emergency department, or ICU experience preferred.
- Strong communication and interpersonal skills, particularly when working with patients and families.
