To support the Journey and Palliative program, the full-time Palliative Care Administrative Specialist will manage patient coordination, conduct follow-up calls, and serve as a resource for patients and their families while working remotely. Key responsibilities Coordinate, schedule, and assign patients to providers within the Journey or Palliative programs Conduct follow-up calls to gather data on patient and family responses to treatment plans Serve as the primary contact and advocate for patients, families, and care teams, ensuring compliance with regulations and confidentiality Required qualifications Minimum of 2 years of experience in Palliative, Hospice, or Home Care preferred Bachelor's degree in a health-related field preferred Experience with medical records and HCHB strongly preferred Proven data analysis skills with experience using electronic health records Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint
